ENROLLMENT, TUITION AND FEES

ENROLLMENT (FOR FULL AND PART-TIME CARE ONLY)

OUR ENROLLMENT PROCESS IS AS FOLLOWS:

  1. ALL PARENTS SEEKING TO ENROLL THEIR CHILDREN IN OUR FIRST LOVE MUST COMPLETE A TOUR OF THE FACILITY WHERE THEY CAN RECEIVE PERTINENT INFORMATION REGARDING THE CENTER’S PROGRAMMING, PROCEDURES AND PROTOCOL, AS WELL AS SEE THEIR CHILD’S CLASSROOM, AND MEET THEIR CHILD’S TEACHER(S) AND PEERS.

  2. ALL APPROPRIATE PAPERWORK WILL BE ADMINISTERED AT THIS TOUR. IT IS RECOMMENDED THAT THE PAPERWORK BE FILLED OUT IN ITS ENTIRETY DURING THIS VISIT, THOUGH PARENTS DO HAVE THE OPTION TO TAKE THE PAPERWORK WITH THEM TO COMPLETE AT THEIR LEISURE. KEEP IN MIND, HOWEVER, THAT YOUR CHILD WILL NOT BE PERMITTED TO ATTEND CHILDCARE UNTIL SUCH TIME AS THE FOLLOWING DOCUMENTATION IS RECEIVED COMPLETED IN FULL IN THE OFFICE:

  • ODJFS 01234 CHILD ENROLLMENT AND HEALTH INFORMATION FOR CHILDCARE

  • ODJFS 01218 BASIC INFANT INFORMATION FOR CHILDCARE (AS APPLICABLE)

  • ODJFS 01305 CHILD MEDICAL STATEMENT FOR CHILDCARE (NOT APPLICABLE TO SCHOOL AGED CHILDREN)

  • ODJFS 01236 CHILD MEDICAL/PHYSICAL CARE PLAN FOR CHILD CARE (AS APPLICABLE)

  • ODJFS 01217 REQUEST FOR ADMINISTRATION OF MEDICATION FOR CHILDCARE (AS APPLICABLE)

  • ODJFS 01511 FAMILY INFORMATION FOR STEP UP TO QUALITY PROGRAMS

  • AN OUR FIRST LOVE TRANSPORTATION CONTRACT OR ODJFS 01225 ROUTINE TRIP PERMISSION FOR CHILDCARE FORM (AS APPLICABLE)

  • AN APPROPRIATE AGES AND STAGES QUESTIONAIRE FOR CHILDREN 2 MONTHS THROUGH 60 MONTHS

  • CUSTODIAL AGREEMENTS/ARRANGEMENTS (AS APPLICABLE AND SANCTIONED BY THE COURT)

  • A LIST OF PERSONS 18 YEARS OF AGE AND OLDER TO WHOM YOU WILL GRANT PERMISSION TO PICK-UP YOUR CHILD (THIS INFORMATION SHOULD INCLUDE THE PERSON’S FULL NAME, PHONE NUMBER, AND RELATIONSHIP TO THE CHILD)- A PHOTOCOPY OF THE PERSON’S I.D. WILL BE TAKEN SHOULD THEY ARRIVE TO TAKE CUSTODY OF THE CHILD- THE CHILD WILL NOT BE RELEASED TO THIS PERSON WITH OUT THESE REQUIREMENTS BEING MET!)

  1. AN ACCURATE SCHEDULE OF THE DAYS AND TIMES THAT YOUR CHILD/CHILDREN WILL BE IN ATTENDANCE IS REQUIRED (AT THE TIME OF THE TOUR IT WILL BE DETERMINED AS TO THE SHIFT FOR WHICH YOU WILL BE REQUIRING CARE (1ST, 2ND, 3RD, MID, VARIABLE). IF YOU NEED A SHIFT CHANGE, THIS MUST BE CLEARED WITH THE ADMINISTRATOR BEFORE YOU WILL BE ALLOWED TO USE HOURS ON THAT SHIFT). THIS SCHEDULE SHOULD BE TURNED IN WEEKLY AND DETAIL THE DAYS THAT YOU WILL REQUIRE CARE, GIVING THE EXACT DROP-OFF AND PICK-UP TIMES. AS PERTAINS TO SCHEDULING, PLEASE OBSERVE THE FOLLOWING:

  • SCHEDULES ARE DUE WEEKLY BY SATURDAY 4PM- EVEN IF YOUR SCHEDULE WILL BE STAYING THE SAME, TURNING YOUR SCHEDULE IN BY THE DESIGNATED TIME CONFIRMS THAT YOUR CHILDREN WILL BE IN ATTENDANCE FOR THE UPCOMING WEEK, AND GUARANTEES THAT YOU WILL RECEIVE CARE WHEN REQUESTED. PARENTS ARE RESPONSIBLE FOR WRITING DOWN THEIR OWN SCHEDULES ON THE WEEKLY SCHEDULE SHEET PROVIDED BY OUR STAFF.

  • SCHEDULE CHANGES MAY ONLY BE ACCOMMODATED BY MANAGEMENT- WE WILL MAKE EVERY EFFORT TO ACCOMMODATE CHANGES AS REQUESTED, BUT IF AN ACCOMMODATION IS UNABLE TO BE MADE, YOU WILL NEED TO SECURE ALTERNATE CHILDCARE ARRANGEMENTS.

  • WHEN YOU MAKE A SCHEDULE, WE EXPECT THAT IT WILL BE FOLLOWED AS WRITTEN. WHETHER YOU ARE AN LCJFS CLIENT OR A PRIVATELY PAYING CLIENT, YOU FORFEIT HOURS WHEN YOU BRING CHILDREN IN LATE OR FAIL TO BRING THEM IN AT ALL AS SCHEDULED WITHOUT GIVING US PROPER, TIMELY NOTICE. OUR STAFF WILL BE IN PLACE TO PROVIDE CARE AS REQUESTED, WE WILL NOT GUARANTEE THAT YOU CAN MAKE UP HOURS LOST FOR TARDINESS OR ABSENTEEISM, MEANING THAT YOU MAY NOT PUSH YOUR PICK-UP TIME BACK OR HAVE DAYS/HOURS/TUITION/CO-PAYS PRORATED FOR A DIFFERENT TIME WITHOUT CONSENT FROM THE ADMINISTRATOR. THIS STIPULATION IS STRICLY ENFORCED AND IN PLACE TO HELP US TO MAINTAIN COMPLIANCE WITH ODJFS CHILDCARE LICENSING RULES REGARDING MAINTAINING RATIOS.

  • ADDITIONALLY, WE WILL NOT GUARANTEE THAT YOU WILL GET IN EARLIER THAN YOUR RECORDED DROP-OFF TIME. YOU MUST BE SURE TO FACTOR IN TRAVEL TIME WHEN MAKING YOUR SCHEDULE. DO NOT RECORD THE TIME THAT YOU MUST REPORT TO WORK AS THE TIME THAT YOU WILL BE DROPPING OFF YOUR CHILDREN TO THE CENTER ON THE SCHEDULE. IF YOU SHOW UP BEFORE THE TIME THAT YOU HAVE RECORDED, EXPECTING TO GAIN ACCESS TO THE CENTER SO THAT YOU CAN BE TO WORK ON TIME, YOU MAY BE SUBJECT TO DENIED ENTRANCE INTO THE FACILITY. YOU MAY BE ASKED TO WAIT UNTIL YOUR SCHEDULED TIME, OR YOU MAY BE STUCK WAITING OUTSIDE (ESPECIALLY PERTINENT WITH EARLY MORNING DROP-OFF AS NO ONE MAY HAVE ARRIVED AT THE CENTER TO LET YOU IN. WHEN WE SEE 6:00 AM DROP-OFF, WE CANNOT KNOW THAT THAT IS THE TIME THAT YOU ARE TO REPORT TO WORK AND THAT YOU ACTUALLY NEED TO BE LET IN EARLIER TO BE ON TIME. OUR STAFF ARE SCHEDULED ACCORDING TO THE SCHEDULES THAT THE PARENTS PROVIDE; WE CANNOT HOPE TO PROVIDE ACCURATE CARE IF WE DO NOT HAVE AN ACCURATE SCHEDULE.

*PLEASE BE MINDFUL THAT THIS IS NOT A DROP-OFF CENTER. WE OFFER 24-HR CARE, MONDAY THROUGH SATURDAY, AS NEEDED, AND SCHEDULE OUR STAFF ACCORDINGLY. IF WE HAPPEN NOT TO BE NEEDED FOR AN EXTENDED PERIOD OF TIME, WE DO CLOSE OUR DOORS UNTIL SUCH TIME AS A CARE NEED RESUMES. WE ALSO SEND STAFF HOME WHEN NOT NEEDED. IF YOU WILL NOT BE FOLLOWING YOUR SCHEDULE AS RECORDED FOR THE WEEK, YOU NEED TO MAKE US AWARE OF ANY CHANGES AS SOON AS POSSIBLE OR RUN THE RISK THAT YOUR CARE NEED WILL NOT BE MET; WE WILL MAKE EVERY EFFORT TO ACCOMMODATE SCHEDULE CHANGES AS PERMISSIBLE, BUT WE CANNOT ALLOW THE CENTER TO BE OUT OF COMPLIANCE TO MEET YOUR CARE NEEDS!- HENCE, CONSISTENT, TIMELY, AND ACCURATE COMMUNICATION IS THE KEY TO A SUCCESSFUL RELATIONSHIP AND IS THE RESPONSIBILITY OF BOTH THE CENTER AND THE PARENT.

 

ONCE ALL FOUR CRITERIA HAVE BEEN MET, YOUR CHILD IS CONSIDERED ENROLLED.

PLEASE NOTE- OUR FIRST LOVE RESERVES THE RIGHT TO REFUSE CARE/ENROLLMENT TO INDIVIDUALS WHO REFUSE:

  1. THEIR CHILD/CHILDREN RECEIVING CHILDHOOD VACCINATIONS (IN WHOLE OR IN PART).

  2. TO GRANT PERMISSION FOR EMERGENCY SERVICES (EMS- 911) TO BE CALLED FOR THE TRANSPORTING OF THEIR CHILD TO THE NEAREST HOSPITAL IN THE EVENT OF ANY INCIDENT REQUIRING SUCH A RESPONSE.

THIS IS NON-NEGOTIABLE AND STRICTLY ENFORCED
